• Submission of forms DOES NOT guarantee you a spot on the Team. Due to limited resources and safety, we are restricted in the number of athletes we can accommodate. If we are unable to seat you, you will receive a full refund.

• Cost: $600.00 | Competitive/Select​

  •   $450.00 | Club Program​

  •   $350.00 | Novice Program

  • (sibling discount is $50.00 off second child, $50.00 off third child)

• DO NOT SUBMIT THE ORIGINAL VHSL PHYSICAL. SAVE THE ORIGINAL, if you participate in sports at your school you will have to submit this form to the HS. We will NOT be able to provide you with a copy. We will accept a scanned pdf. or you may drop a copy and drop off to MMC Registrar. WE WILL NOT ACCEPT PICTURES, do not photograph your form and email me the pictures.

• *The physical (not just the form- the actual physical) must have been completed after May 1, 2021 and will be good through Spring 2022. We will accept a physical completed in the last year, in the VHSL format.

• Make/keep a copy of your submission for your records. You will be invoiced/notified of amount due when spots have been assigned. You may pay online or make check payable to "MMC" and drop off in black "mail box" on front step:
